A simple form requiring only 7-11 pieces of data to complete.

The project

Atypical providers supporting Long-Term Supports and Services (LTSS) often struggle with standard claims forms, which contain numerous fields and require knowledge of medical coding. As a result, many providers resorted to submitting manual invoices via fax or email, leading to errors, delays, and a cumbersome process for both providers and payers.

What we did

We created a simplified, user-friendly claims form with only 6 required fields and 4 optional inputs. The form was designed to ensure that providers could easily fill it out without the need for medical coding knowledge. We also included guidance for potentially confusing fields to minimize errors and streamline the process. This solution drastically reduced the need for manual submissions and improved payment processing.

The outcome

The new claims form led to significant improvements in the process:

  • Faster, more accurate claims processing, eliminating delays and reducing manual data entry for payers.
  • Less than 1% of claims were submitted manually.
  • A 40% reduction in rejections due to fewer errors and missing information.
